Hazardous Materials Acknowledgement Form and Waiver Agreement

Each customer must read and accept the following conditions for each purchase of lye (sodium hydroxide) and/or caustic potash (potassium hydroxide) and/or potassium carbonate and/or phenolphthalein from Certified Lye™.

I accept and agree to all of these conditions for purchasing lye and/or caustic potash from Certified Lye™.

I do not accept these conditions.

By selecting the "I accept and agree" option, I hereby understand, acknowledge, and agree to the following:

  1. I certify that I am at least 18 years of age.
  2. I am not purchasing this lye (sodium hydroxide) and/or caustic potash (potassium hydroxide) for any illegal use or purpose.
  3. I will use this lye (sodium hydroxide) and/or caustic potash (potassium hydroxide) in a manner compliant with all local, state, and federal laws.
  4. I assume all responsibility for the general possession, the proper storage, and the safe usage of this lye (sodium hydroxide) and/or caustic potash (potassium hydroxide).
  5. I understand that lye (sodium hydroxide) and caustic potash (potassium hydroxide) are hazardous materials and have the potential to be harmful or fatal if misused.
  6. I understand that I am purchasing and using this lye (sodium hydroxide) and/or caustic potash (potassium hydroxide) at my own risk.
  7. I understand that while using this lye (sodium hydroxide) and/or caustic potash (potassium hydroxide) I may encounter unavoidable risks or hazards where injuries or damage may occur.
  8. I agree to release, hold harmless, and indemnify Certified Lye™ from any liability for injury or damage that may occur in connection with the use or misuse of this lye (sodium hydroxide) and/or caustic potash (potassium hydroxide).
  9. I intend this release and indemnification to be effective, regardless the type of legal recovery theory that may be chosen.
  10. I will make no claim or legal action against Certified Lye™ arising from the use or misuse of this lye (sodium hydroxide) and/or caustic potash (potassium hydroxide).
  11. Should litigation arise from the use or misuse of this lye (sodium hydroxide) and/or caustic potash (potassium hydroxide), I agree to hold Certified Lye™ entirely harmless.
  12. If I do not understand or if I disagree with any of these conditions, I will not purchase or use lye (sodium hydroxide) and/or caustic potash (potassium hydroxide) from Certified Lye™.
  13. By selecting the "I accept and agree" option and by purchasing this lye (sodium hydroxide) and/or caustic potash (potassium hydroxide), I certify that I have read all of these conditions, I understand all of these conditions, and I fully agree to and will comply with all of these conditions.
